This survey is for anyone who has lived experience of accessing Adult Social Care, whether that be for yourself or someone you support, either now or in the past.

We want to learn from your experiences and make sure we are providing the right kind of services in the right way for people, their families and their carers. We want to take time to listen to people, families, and carers. The more people who can share their views, the greater the chance we have of working together to improve and design services together.
