Focus Group Consent

Thank you for your interest in taking part in this focus group. Before participating, please read the following statements carefully and indicate your agreement.

To take part in the focus group, you must answer "Yes" to all consent questions. If you select "No" to any of the questions, we will be unable to include you in the session, as we must ensure that everyone taking part has provided full consent to participate and to the information being collected.

Your responses will be treated confidentially and used only for the purposes outlined in this consent form. Please ensure you have read and understood each statement before making your selections.
