(Required) 0 - 4 years 5 - 10 years 11 - 15 years 16 - 19 years 20 - 25 years

3. Does your child or young person currently have an Educational Health Care Plan (EHCP)?

(Required) Yes I have never requested an EHCP for my child/young person I have requested an EHCP and the process is ongoing I have requested an EHCP but was declined issue My child or young person previously had an EHCP but it is no longer in place
